       Introduced  by  Sen.  HANNON -- read twice and ordered printed, and when
         printed to be committed to the Committee on Health
       AN ACT to amend the social services law, in relation  to  directing  the
         commissioner  of  health  to develop a uniform prior prescription drug
         authorization form for managed care providers
         THE PEOPLE OF THE STATE OF NEW YORK, REPRESENTED IN SENATE AND  ASSEM-
       BLY, DO ENACT AS FOLLOWS:
    1    Section  1.  Section  364-j  of  the social services law is amended by
    2  adding a new subdivision 26 to read as follows:
    3    26. ON OR BEFORE JANUARY FIRST, TWO THOUSAND THIRTEEN, THE COMMISSION-
    4  ER OF HEALTH SHALL DEVELOP A UNIFORM  PRIOR  AUTHORIZATION  FORM  TO  BE
    5  UTILIZED  BY  ALL  MANAGED  CARE  PROVIDERS  FOR PURPOSES OF DETERMINING
    6  COVERAGE OF PRESCRIPTION DRUG BENEFITS UNDER THIS TITLE.
    7    S 2. This act shall take effect immediately; provided,  however,  that
    8  the  amendment  to  section  364-j  of  the social services law, made by
    9  section one of this act shall not affect the repeal of such section  and
   10  shall be deemed repealed therewith.
