STATE OF HAWAII

A BILL FOR AN ACT

 

 

RELATING TO HUMAN SERVICES.

 

 

BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF HAWAII:

 

     SECTION 1.  Chapter 346, Hawaii Revised Statutes, is amended by adding a new section to be appropriately designated and to read as follows:

     "§346-    Advance health-care directive form; medicaid.  The department of human services shall develop an advance health care form and make the form available to each medicaid recipient.  The recipient may submit a completed form to the recipient's primary care provider."

     SECTION 2.  New statutory material is underscored.

     SECTION 3.  This Act shall take effect upon its approval.

 

Report Title:

Advance Health Care Directive; Medicaid

 

Description:

Requires the department of human services to develop an advance health care directive form and make the form available to each medicaid recipient.  Allows the medicaid recipient to submit a completed advance health care directive form to the recipient's primary care provider.  (SD1)
