Summary

Create a new section of KRS Chapter 205 to establish the Hospital-to-Home Transition Program to provide temporary services to elderly and disabled persons when they are discharged from an institutional setting to their home or a community setting; require an applicant to submit a completed application; require the cabinet to make an eligibility determination within 60 days; require the Department for Medicaid Services to submit an application for a waiver to the Centers for Medicare and Medicaid Services; require the cabinet to request funding and begin the program by July 1, 2016, subject to funding; require the department to promulgate administrative regulations.
