Summary
To require that covered health care services provided by out-of-network providers at in-network hospitals be covered at an in-network level, including an in-network level of cost sharing, prohibit balance billing for such services and require that each out-of-network provider that provides such services accept, as payment in full for such services, payment for such services at the in-network rate.
