Bill Title: Amends the Mental Health and Developmental Disabilities Code. Repeals provisions requiring the Department of Human Services to prescribe all forms necessary for proceedings concerning (i) the admittance of a person as an inpatient to a mental health facility for treatment of mental illness and (ii) the admittance of a person with a developmental disability to a facility for residential and habilitation services. Effective immediately.

AN ACT concerning health.
| ||||
Be it enacted by the People of the State of Illinois,
| ||||
represented in the General Assembly:
| ||||
Section 5. The Mental Health and Developmental | ||||
Disabilities Code is amended by changing Sections 3-201 and | ||||
4-202 as follows:

Sec. 3-201.
The Department shall prescribe all forms | ||||
necessary for
proceedings under this Chapter, and all forms | ||||
used in such proceedings shall
comply substantially with the | ||||
forms so prescribed. The Department shall
publish all forms in | ||||
electronic format and post the forms to its website print and | ||||
furnish an initial supply of such forms to the clerks of the | ||||
circuit courts .
