AN ACT
Amending title 32, chapter 18, article 3, Arizona Revised Statutes, by adding section 32-1978; relating to the state board of pharmacy.

Be it enacted by the Legislature of the State of Arizona:
Section 1. Title 32, chapter 18, article 3, Arizona Revised Statutes, is amended by adding section 32-1978, to read:
32-1978. Refusal to fill certain prescriptions; disclosure; requirements
A pharmacist who is licensed pursuant to this chapter and who refuses to fill certain prescription orders or to stock certain medications because of the pharmacist's moral or religious beliefs must file a disclosure form as prescribed by the department of health services. The disclosure form must include the pharmacist's name and license number and the name and address of the pharmacist's employer. The department shall post each disclosure on its website to provide public notification of the pharmacist's policy.
