Summary

Requires private health insurance plans to include on insurance membership cards the hospital nearest to the primary residence of the plan holder, contact information for customer service, deductible amount applicable to the plan holder, out-of-pocket maximum applicable to the plan holder, and cost-sharing amount for in-network emergency or urgent care services applicable to the plan holder.
