AVELLA -- read twice and ordered printed, and when printed to be committed to the Committee on Social Services -- commit- tee discharged and said bill committed to the Committee on Rules -- reported favorably from said committee to third reading, amended and ordered reprinted, retaining its place in the order of third reading -- passed by Senate and delivered to the Assembly, recalled, vote reconsidered, restored to third reading, amended and ordered reprinted, retaining its place in the order of third reading AN ACT to amend the social services law and the not-for-profit corpo- ration law, in relation to inquiring into past military service on applications for public welfare The People of the State of New York, represented in Senate and Assem- bly, do enact as follows: 1 Section 1. Subdivision 3 of section 20 of the social services law is 2 amended by adding a new paragraph (j) to read as follows: 3 (j) to ensure the provision, on any form required to be completed at 4 application or recertification for the purpose of obtaining financial 5 assistance pursuant to this chapter, the form shall contain a check-off 6 question asking whether the applicant or recipient or a member of his or 7 her family served in the United States military, and an option to answer 8 in the affirmative. Where the applicant or recipient answers in the 9 affirmative to such question, the office of temporary and disability 10 assistance shall ensure that contact information for the state division 11 of veterans' affairs is provided to such applicant or recipient, in 12 addition to any other materials provided. 13 § 2. Section 202 of the not-for-profit corporation law is amended by 14 adding a new paragraph (g) to read as follows: 15 (g) Every corporation receiving any kind of state funding shall ensure 16 the provision on any form required to be completed at application or 17 recertification for the purpose of obtaining financial assistance pursu- 18 ant to this chapter, that the application form shall contain a check-off 19 question asking whether the applicant or recipient or a member of his or 20 her family served in the United States military, and an option to answer 21 in the affirmative. This act shall take effect on the sixtieth day after it shall 6 have become a law.
