          read once and referred to the Committee on Health

        AN ACT to amend the public health law, in relation to including informa-
          tion  on  if a person previously lived in a nursing home on his or her
          death certificate

          The People of the State of New York, represented in Senate and  Assem-
        bly, do enact as follows:

     1    Section  1.  Subdivision 4 of section 4141 of the public health law is
     2  amended by adding a new paragraph (e) to read as follows:
     3    (e) Every certificate shall include, in a separate  area,  information
     4  on  if the decedent previously resided in a nursing home issued an oper-
     5  ating certificate as a nursing home pursuant to article twenty-eight  of
     6  this  chapter.  Such  information  shall be indicated on the certificate
     7  including, if known to the person completing the death certificate,  the
     8  name of the nursing home, location of the nursing home, and the approxi-
     9  mate dates in which the decedent resided in the nursing home.
    10    §  2.  This  act shall take effect on the thirtieth day after it shall
    11  have become a law.
