Bill Title: Creates the Patient Intake Form Military Veteran Inquiry Act. Provides that a health care provider must include on each patient intake form a question asking whether the patient or the patient's spouse has ever served in the military for purposes of tailoring treatment options. Defines "health care provider" and "military veteran".
