1 AN ACT concerning health.
2 Be it enacted by the People of the State of Illinois,
3represented in the General Assembly:
4 Section 1. Short title. This Act may be cited as the
5Patient Intake Form Military Veteran Inquiry Act.
6 Section 5. Purpose. The purpose of this Act is to improve
7the tailoring of treatment options for military veterans.
8 Section 10. Definition. As used in this Act:
9 "Health care provider" means any physician or public or
10private facility that provides, on an inpatient or outpatient
11basis, preventive, diagnostic, therapeutic, convalescent,
12rehabilitation, mental health, or intellectual disability
13services, including general or special hospitals, skilled
14nursing homes, extended care facilities, intermediate care
15facilities, and mental health centers.
16 "Military veteran" means a person who has served in any
17component of the U.S. Armed Forces, including reservists;
18members of the National Guard of the United States; or the
19National Guard of any state, the District of Columbia, a
20commonwealth, or a territory of the United States.
