Summary

Requires health care providers to provide opportunity for child's parent or legal guardian to provide contact information so that he or she may receive information from specified service providers when hearing loss is identified; requires DOH to register certain service providers & institutions; allows parent or legal guardian to request services from participating service provider; provides that level of services received is based on child's individualized education program or individual & family service plan; provides for eligibility; provides funding formula; requires department to develop standards for participating service providers; authorizes department to adopt rules.
