       the following:
       
    1         Senate Amendment (with title amendment)

    3         Delete lines 113 - 114
    4  and insert:
    5         8. A small employer carrier must limit changes to
    6  prescription drug formularies as required by s. 627.42393.

   10         Delete lines 8 - 9
   11  and insert:
