Summary

This bill requires the Department of Education to provide, in schools that offer health services to children at a School-Based Health Center (SBHC), consent forms that clearly permit a parent or guardian to exclude any health service or services that the parent or guardian does not want his or her child to receive at the SBHC, and clearly explain that withholding permission for a child to receive any particular service or services does not prevent the child from obtaining all other services offered by the SBHC.
