AN ACT CONCERNING THE COLLECTION OF DATA BY HEALTH AND HUMAN SERVICES AGENCIES.

Be it enacted by the Senate and House of Representatives in General Assembly convened:

Section 1. (NEW) (Effective January 1, 2015) The Department of Public Health, the Department of Social Services and any other office or agency of the state that provides services relating to health or human services shall collect primary data concerning the identity of persons served by such department, office or agency including such persons' race, ethnicity, gender, primary language, disability status and income, provided such data shall be aggregated in a manner that does not identify such persons. Such department, office or agency may collect additional data, including, but not limited to, the residential address, socioeconomic status and level of acculturation of such persons. Any such data shall be collected by such departments, office or agency using a self-reporting method, except a parent or guardian may report such information by responding to questions on behalf of a minor and a guardian or conservator may report such information by responding to questions on behalf of a person who is incapacitated.

This act shall take effect as follows and shall amend the following sections:

To require certain state agencies to collect and maintain identifying information concerning the persons such agencies serve.
