AN ACT CONCERNING THE DEVELOPMENT AND USE OF A UNIVERSAL CHILD HEALTH FORM.

Be it enacted by the Senate and House of Representatives in General Assembly convened:

Section 1. (Effective from passage) (a) The Commissioner of Public Health, in consultation with the Commissioner of Education, shall (1) develop a single child health form to be used by schools, youth camps, day care homes and child care centers to obtain health information and authorization for a child to attend or participate in activities at the school or facility, and (2) develop a plan for implementation of the use of such form.

(b) Not later than October 1, 2013, the Commissioner of Public Health shall report, in accordance with the provisions of section 11-4a of the general statutes, to the joint standing committees of the General Assembly having cognizance of matters relating to public health and education concerning the contents of such form and the commissioner's plan to implement the use of such form.

This act shall take effect as follows and shall amend the following sections:
