Summary
To require health insurance coverage for mammograms, not less frequently than once annually and upon the recommendation of a licensed physician, for each covered woman who has (1) a personal or family history of breast cancer, (2) a personal history of breast disease diagnosed through biopsy as benign, or (3) not given birth before attaining thirty years of age.
