AN ACT

 

Amending Title 23, chapter 2, article 7, Arizona Revised Statutes, by adding section 23-351.01; relating to payment of wages.

Be it enacted by the Legislature of the State of Arizona:

Section 1.  Title 23, chapter 2, article 7, Arizona Revised Statutes, is amended by adding section 23-351.01, to read:

START_STATUTE23-351.01.  Payment of wages; total job compensation statement

At the time of payment of wages, an employer shall provide to each employee a total job compensation statement.  The total job compensation statement shall be in writing, shall be either a detachable part of the check, draft or voucher or a separate document if the wages are paid by personal check or cash and shall include an itemized list of all of the following:

1.  Gross wages earned.

2.  Net wages earned.

3.  The employer's and employee's cost for the following benefits:

(a)  Medical.

(b)  Life insurance.

(c)  Long-term disability insurance.

(d)  Retirement.

(e)  Social security.

(f)  Medicare.

(g)  Unemployment insurance.

(h)  Workers' compensation. END_STATUTE
