Summary

As introduced, requires a certificate of birth for each live birth that occurs in this state to include a separate box for the institution, physician, or healthcare provider that prepares the certificate to write in the mother and father's social security number or taxpayer identification number (TIN); provides that if no social security number or TIN is provided, then the institution, physician, or healthcare provider must write in the box "Not provided". - Amends TCA Title 36; Title 63 and Title 68.
