STATE OF NEW YORK ________________________________________________________________________ 7454 2021-2022 Regular Sessions IN ASSEMBLY May 10, 2021 ___________ Introduced by M. of A. JACKSON -- read once and referred to the Commit- tee on Health AN ACT to amend the public health law, in relation to the maternal health crisis The People of the State of New York, represented in Senate and Assem- bly, do enact as follows: 1 Section 1. This act shall be known and may be cited as the "Chisholm 2 Chance Act". 3 § 2. Title 3 of article 25 of the public health law is REPEALED and a 4 new title 3 is added to read as follows: 5 TITLE III 6 CHISHOLM CHANCE ACT 7 Section 2560. Legislative findings. 8 2561. Administrative hub. 9 § 2560. Legislative findings. 1. The legislature hereby finds and 10 declares that severe maternal morbidity constitutes a maternal health 11 crisis that poses a serious threat to the health and quality of life of 12 many women and children. New York is among the bottom third of states 13 for the number of women dying, with women of color dying at a rate three 14 and one-half times more than other women. The highest rates of severe 15 maternal morbidity occurs in two counties, Kings and Bronx county, with 16 the highest rate occurring in north and central Brooklyn. 17 2. The legislature further finds and declares that a plan should be 18 established to address the severe maternal morbidity crisis. 19 § 2561. Administrative hub. 1. The department shall develop, establish 20 and administer a community led and operated administrative hub in Kings 21 county and Bronx county to contract with multiple payers in order to 22 coordinate outreach by specially trained community-based organizations 23 and community health workers to find and engage the highest need women 24 and children in Kings county and Bronx county. LBD02494-01-1A. 7454 2 1 (a) establish the administrative hub's infrastructure; 2 (b) establish an advisory board and governance structure for the 3 administrative hub; 4 (c) hire and train community health workers and doulas, up to one per 5 community-based organization; 6 (d) establish a single point of entry for addressing social determi- 7 nants of health; 8 (e) synchronize systems across networks of community-based organiza- 9 tions with healthcare, behavioral and social service delivery systems; 10 and 11 (f) achieve certification from the Pathways Community Hub Institute 12 within six months of the beginning of operations. 13 2. Community health workers shall be contracted to identify and 14 address standardized health risks via shared metrics or standardized 15 pathways that have demonstrated an impact on the social determinants of 16 health. 17 3. The administrative hub shall coordinate participating community- 18 based organizations and community health workers to address gaps in the 19 systems of care for the highest need women and children in Kings county 20 and Bronx county. 21 4. The administrative hub shall begin operations before the first of 22 January next succeeding the effective date of this title. 23 § 3. This act shall take effect immediately.
