       AN  ACT  to amend the public health law, in relation to requiring hospi-
         tals to provide forms in Punjabi
         THE PEOPLE OF THE STATE OF NEW YORK, REPRESENTED IN SENATE AND  ASSEM-
       BLY, DO ENACT AS FOLLOWS:
    1    Section  1.  The  public health law is amended by adding a new section
    2  2805-x to read as follows:
    3    S 2805-X. FORMS  PRINTED  IN  PUNJABI.  EVERY  HOSPITAL  SHALL,  AT  A
    4  PATIENT'S REQUEST, PROVIDE ANY MEDICAL FORM OR PRINTED INFORMATION WHICH
    5  IS  GIVEN  TO  A PATIENT OR REQUIRED TO BE SIGNED BY SUCH PATIENT TRANS-
    6  LATED INTO PUNJABI.
    7    S 2. This act shall take effect on the ninetieth day  after  it  shall
    8  have become a law.
