Bill Title: Amends the Unified Code of Corrections. Provides that a committed person who is at least 55 years of age and who has served at least 25 consecutive years of imprisonment in a Department of Corrections institution or facility may petition the Prisoner Review Board for participation in the Sentence Modification Program. Provides that a committed person who is serving a sentence, including one who has not yet served the minimum term of the sentence, who is diagnosed as suffering from a terminal condition so as to render the committed person likely to live less than 9 months may be released on medical parole to a hospital, hospice, other licensed inpatient facility, or suitable housing accommodation as specified by the Board. Establishes eligibility requirements for the Program. Provides that an offender who meets the criteria established by this provision and the Department shall be considered by the Department for a reduction of up to 40% of his or her sentence. Provides that after 5 years of successful completion of the Program, the participant may apply to the Board for executive clemency by the Governor, requesting that his or her status be changed to parole or mandatory supervised release or that his or her participation in the Program be extended another 5 years. Retains parole and mandatory supervised release for those not selected for the Program.
