A bill to be entitled
2An act relating to voter information cards; amending s.
397.071, F.S.; requiring that voter information cards
4contain the address of the polling place of the registered
5voter; requiring a supervisor of elections to issue a new
6voter information card to a voter upon a change in a
7voter's address of legal residence or a change in a
8voter's polling place address; providing instructions for
9implementation by the supervisors of elections; providing
10an effective date.

12Be It Enacted by the Legislature of the State of Florida:

14     Section 1.  Section 97.071, Florida Statutes, is amended to
15read:
16     97.071  Voter information card.-
17     (1)  A voter information card shall be furnished by the
18supervisor to all registered voters residing in the supervisor's
19county. The card must contain:
20     (a)  Voter's registration number.
21     (b)  Date of registration.
22     (c)  Full name.
23     (d)  Party affiliation.
24     (e)  Date of birth.
25     (f)  Address of legal residence.
26     (g)  Precinct number.
27     (h)  Polling place address.
28     (i)(h)  Name of supervisor and contact information of
29supervisor.
30     (j)(i)  Other information deemed necessary by the
31supervisor.
32     (2)  A voter may receive a replacement voter information
33card by providing a signed, written request for a replacement
34card to a voter registration official. Upon verification of
35registration, the supervisor shall issue the voter a duplicate
36card without charge.
37     (3)  In the case of a change of name, address of legal
38residence, polling place address, or party affiliation, the
39supervisor shall issue the voter a new voter information card.
40     Section 2.  The supervisor must meet the requirements of
41this act for any elector who registers to vote or who is issued
42a new voter information card pursuant to s. 97.071(2) or (3),
43Florida Statutes, on or after September 1, 2011.
44     Section 3.  This act shall take effect July 1, 2011.
