       Senator Stargel moved the following:
       
    1         Senate Amendment (with title amendment)
    2  
    3         Delete line 75
    4  and insert:
    5  child included in the immunization registry. Each consent to
    6  treatment form provided by a health care practitioner or by an
    7  entity that administers vaccinations or causes vaccinations to
    8  be administered to children from birth through 17 years of age
    9  must contain a notice stating that the parent or guardian of a
   10  child may refuse to have his or her child included in the
   11  immunization registry. The parent or

   15         Delete line 9
   16  and insert:
   17         registry; requiring a specified consent to treatment
