AN ACT TO AMEND TITLE 16 OF THE DELAWARE CODE RELATING TO ESTABLISHING A YELLOW DOT MEDICAL INFORMATION ALERT PROGRAM.


BE IT ENACTED BY THE GENERAL ASSEMBLY OF THE STATE OF DELAWARE:


Section 1.Amend §9706, Chapter 97, Title 16, Delaware Code by making insertions as shown by underlining as follows:

(i) The Director of the Division of Public Health shall create a "Yellow Dot" medical information alert program.

(1) The purpose of this program is to alert emergency medical service providers, police, firefighters, and other emergency service providers of a person's vital medical information if there is an emergency, accident, or otherwise, and the person is in their motor vehicle.

(2) A yellow dot folder to contain vital personal and medical information forms, to be kept in the glove compartment, is to be created.This information shall include such information as:

A. name;

B. address;

C. a photograph;

D. phone number(s) of person(s) to be contacted;

E. medical history;

F. allergies;

G. medications and dosage; and

H. doctor names and phone numbers.

(3) A yellow dot sticker shall be created to be placed in the lower left corner of the rear window of the motor vehicle to alert emergency personnel that vital medical information can be found in the glove compartment of the vehicle.

Section 2.This act shall become effective 180 days after enactment.
