AN ACT CONCERNING HEALTH INSURANCE CLAIM FORMS AND THE DEVELOPMENT OF A UNIFORM TREATMENT AUTHORIZATION FORM FOR MENTAL HEALTH SERVICES.
Be it enacted by the Senate and House of Representatives in General Assembly convened:
That the general statutes be amended to allow health care providers to use his or her national provider identification number on Health Care Financing Administration Fifteen Hundred health insurance claim forms, and to direct the development of a uniform treatment authorization form for mental health services.
Statement of Purpose:
