AN ACT ELIMINATING THE INCLUSION OF A PHYSICIAN'S PERSONALLY IDENTIFIABLE INFORMATION FROM HEALTH INSURANCE CLAIM FORMS.
Be it enacted by the Senate and House of Representatives in General Assembly convened:
That section 38a-477 of the general statutes be amended to: (1) Eliminate the use of a physician's Social Security number or federal tax identification number on standardized health insurance claim forms; and (2) specifically authorize a physician to use his or her national provider identification number on such claim forms in lieu of a Social Security number or federal tax identification number.
Statement of Purpose:
To protect the confidentiality of a physician's Social Security number and federal tax identification number.
