A BILL TO BE ENTITLED
 
AN ACT
  relating to information provided by a voter to an election officer.
         BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF TEXAS:
         SECTION 1.  Chapter 1, Election Code, is amended by adding
  Section 1.021 to read as follows:
         SECTION 1.021
    INFORMATION PROVIDED BY VOTER.  (a)  Any form
  completed by a voter and provided to an election officer shall
  include the voter's name, address, date of birth, voter unique
  identifier (VUID), if available, and the number from either a
  driver's license, election identification certificate, or personal
  identification card issued to the person by the Department of
  Public Safety or last four digits of their social security number.
         (b)  A person desiring to register to vote who does not
  possess the information required by Section 13.002(c)(8)(A) or (B)
  may provide a statement under Section 13.002(c)(8)(C).
         SECTION 2.  This Act takes effect January 1, 2018.
