1.1A bill for an act
1.2relating to real property; providing homeowners facing foreclosure with
1.3information about the current owner of the mortgagee interest in the property, the
1.4current holder of the mortgage, and the third-party servicer of the mortgage loan
1.5if any;amending Minnesota Statutes 2012, section 580.022, subdivision 2.
1.6BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF MINNESOTA:

1.7    Section 1. Minnesota Statutes 2012, section 580.022, subdivision 2, is amended to read:
1.8    Subd. 2. Notice of Counseling and Request for Contact Information form. The
1.9notice required in section 580.021, subdivision 4, must be substantially in the following
1.10form:
1.11"PREFORECLOSURE NOTICE
1.12NOTICE OF PROVISION OF FORECLOSURE PREVENTION COUNSELING
1.13AND REQUEST FOR MORTGAGEE CONTACT INFORMATION
1.14[Insert agency name] has been contacted by your customer regarding foreclosure
1.15prevention counseling in response to the current foreclosure proceedings involving the
1.16customer's real property. Please provide the following contact information pursuant to
1.17Minnesota Statutes, section 580.021, subdivision 4, by completing and returning this form
1.18via fax [insert fax number] or via e-mail at [insert e-mail address].
