| 2 | WHEREAS, The members of the Illinois House of | ||||||
| 3 | Representatives are saddened to learn of the death of Tommie | ||||||
| 4 | Trevion Forest, III of Peoria, who passed away on October 10, | ||||||
| 5 | 2015; and
| ||||||
| 6 | WHEREAS, Tommie Forest was born in Peoria on October 18, | ||||||
| 7 | 2000 to Tommie, Jr. and Rebecca (nee Jordan) Forest; and
| ||||||
| 8 | WHEREAS, Tommie Forest was a freshman at Peoria Central | ||||||
| 9 | High, where he was an honor student and a star basketball and
| ||||||
| 10 | football player; he was a member of the AAU Basketball Team of | ||||||
| 11 | Peoria and the Chicago Mac
Irving Fire AAU Basketball Team; and
| ||||||
| 12 | WHEREAS, Tommie Forest was a member of Pleasant Grove | ||||||
| 13 | Missionary Baptist Church, where he served on
the Junior Usher | ||||||
| 14 | Board; and
| ||||||
| 15 | WHEREAS, Tommie Forest was a kind and loving young man who | ||||||
| 16 | loved his family and had a desire
to play sports and achieve | ||||||
| 17 | success by going to college; and
| ||||||
| 18 | WHEREAS, Tommie Forest was preceded in death by his father, | ||||||
| 19 | Tommie Forest, Jr.; his
grandfather, Tommie Smith; and his | ||||||
| 20 | uncle, Terry Forest; and
| ||||||

| 1 | WHEREAS, Tommie Forest is survived by his mother, Rebecca | ||||||
| 2 | Forest (his
mother's fiance and his special mentor, Marcus | ||||||
| 3 | Moore); his 2 brothers, DeMarcus Pate and
DeVonte Keeton; his 2 | ||||||
| 4 | sisters, Tiamber and Teonna Forest; his grandparents, Dorcus | ||||||
| 5 | Jordan, Gloriastine (Andrew) Lucas, and James (Sandra) Jordan; | ||||||
| 6 | his god-parents,
William (Shirley) Noel; his special friend and | ||||||
| 7 | mentor, Shaun Livingston; and a host of uncles, aunts,
other | ||||||
| 8 | family, and friends; therefore, be it

| 9 | RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE | ||||||
| 10 | NINETY-NINTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we | ||||||
| 11 | mourn the passing of Tommie Trevion Forest, III, and extend our | ||||||
| 12 | sincere condolences to his family, friends, and all who knew | ||||||
| 13 | and loved him; and be it further
