Bill Title: Amends the Mental Health and Developmental Disabilities Code. Provides that the Department of Human Services shall develop a monitoring program for any client that is discharged from a developmental disability facility under the Admission, Transfer, and Discharge Procedures for the Developmentally Disabled Chapter of the Code to a facility licensed under the Community-Integrated Living Arrangements Licensure and Certification Act. Provides that the program shall include weekly face-to-face visits for the first 8 weeks after discharge with an employee of the Department and monthly face-to-face visits after the first 8 weeks with an employee of the Department for one year after discharge.

2 Be it enacted by the People of the State of Illinois,
3represented in the General Assembly:
4 Section 5. The Mental Health and Developmental
5Disabilities Code is amended by adding Section 4-710 as
6follows:

8 Sec. 4-710. Department monitoring program. The Department
9shall develop a monitoring program for any client that is
10discharged from a developmental disability facility under this
11Chapter to a facility licensed under the Community-Integrated
12Living Arrangements Licensure and Certification Act. The
13program shall include weekly face-to-face visits with an
14employee of the Department for the first 8 weeks after
15discharge and monthly face-to-face visits with an employee of

17discharge.
