Bill Title: Amends the Illinois Public Aid Code. Requires the Department of Healthcare and Family Services to establish and implement a managed primary care demonstration project to provide primary care services that are focused on preventive rather than curative care to persons who reside in underserved communities that lack accessible health and medical services. Provides that the demonstration project shall operate for a 5-year period and provide supplemental services to medical assistance recipients, including those who are enrolled in the State's managed care medical assistance program. Requires the Department to contract with a health care organization that is capable of providing patient-centered, prevention-focused services, including, but not limited to, the following: (i) patient navigators to manage patient care; (ii) patient-tailored preventive health care plans; (iii) administrative personal health care consultants for home health maintenance between medical office visits; (iv) clinical personal health care consultants for telehealth; (v) an online "virtual" health hub that provides patients with access to wellness, self-guided education, health seminars, and additional health and wellness resources; (vi) community health and human services centers to engage, educate, and empower patients to get involved in their own self-care; (vii) mobile preventive health stations and kiosks; and (viii) call centers to interact with medical homes and facilitate service offerings. Provides that the demonstration project shall be implemented no later than 6 months after the effective date of the amendatory Act. Effective immediately.

1 AN ACT concerning public aid.
2 Be it enacted by the People of the State of Illinois,
3represented in the General Assembly:
4 Section 5. The Illinois Public Aid Code is amended by
5adding Section 12-4.57 as follows:
6 (305 ILCS 5/12-4.57 new)
7 Sec. 12-4.57. Managed Primary Care Demonstration Project.
8The Department shall establish and implement a Managed Primary
9Care Demonstration Project to provide primary care services
10that are focused on preventive rather than curative care to
11persons who reside in underserved communities that lack
12accessible health and medical services. The demonstration
13project shall operate for a 5-year period and provide
14supplemental services to medical assistance recipients,
15including those who are enrolled in the State's managed care
16medical assistance program. The Department shall contract with
17a health care organization that is capable of providing
18patient-centered, prevention-focused services, including, but
19not limited to, the following:
20 (1) Patient navigators to manage patient care.
21 (2) Patient-tailored preventive health care plans.
22 (3) Administrative personal health care consultants
23 for home health maintenance between medical office visits.

2 telehealth (health information and advice) and wellness
3 initiatives.
4 (5) A patient portal.
5 (6) An online "virtual" health hub that provides
6 patients with access to wellness, self-guided education,
7 health seminars, a video library, and additional health
8 and wellness resources.
9 (7) Community health and human services centers to
10 engage, educate, and empower patients to get involved in
11 their own self-care.
12 (8) Mobile preventive health stations and kiosks to
13 bring services to underserved communities that are "health
14 or medical deserts".
15 (9) Call centers to interact with medical homes and
16 facilitate service offerings.
17 The demonstration project shall be implemented no later
18than 6 months after the effective date of this amendatory Act
19of the 103rd General Assembly.
20 Section 99. Effective date. This Act takes effect upon
21becoming law.
