| 1 | AMENDMENT TO HOUSE BILL 5463

| 2 | AMENDMENT NO. ______. Amend House Bill 5463 by replacing | ||||||
| 3 | everything after the enacting clause with the following:
| ||||||
| 4 | "Section 5. The Mental Health and Developmental | ||||||
| 5 | Disabilities Administrative Act is amended by changing Section | ||||||
| 6 | 53 as follows:
| ||||||
| 7 | (20 ILCS 1705/53) (from Ch. 91 1/2, par. 100-53)
| ||||||
| 8 | Sec. 53.
The Department shall create a consistent case | ||||||
| 9 | coordination system
for persons with a developmental | ||||||
| 10 | disability who receive services provided or
funded by the | ||||||
| 11 | Department. The objectives of this system shall be to ensure | ||||||
| 12 | that
a full range of an individual's needs is identified and | ||||||
| 13 | assessed through
statewide use of an individual client | ||||||
| 14 | assessment tool; to ensure that each
individual actually | ||||||
| 15 | receives, in the most effective and efficient combination
and | ||||||
| 16 | sequence, the full range and continuum of services needed; to | ||||||
| |||||||
| |||||||
| 1 | ensure that
all available resources are applied appropriately | ||||||
| 2 | to each individual served;
and to provide a systematic | ||||||
| 3 | procedure for serving individuals which generates
among and | ||||||
| 4 | within the local service delivery agencies information | ||||||
| 5 | required for
effective system management.
| ||||||
| 6 | Each individual residing in a community integrated living | ||||||
| 7 | arrangement shall receive an annual assessment to screen that | ||||||
| 8 | individual for any health issues or risks. Beginning July 1, | ||||||
| 9 | 2019, each individual shall receive his or her annual client | ||||||
| 10 | assessment via a web-based, electronic screening tool. The | ||||||
| 11 | electronic screening tool shall replace the current | ||||||
| 12 | paper-based assessment. A provider may make a request, along | ||||||
| 13 | with justifications, to the Department to complete the | ||||||
| 14 | assessment on paper. Subject to appropriation, the Department | ||||||
| 15 | may contract with a third-party entity to create and implement | ||||||
| 16 | the web-based, electronic screening tool. The Department shall | ||||||
| 17 | make changes to its rules in the Illinois Administrative Code | ||||||
| 18 | to incorporate a web-based, electronic assessment tool. | ||||||
| 19 | "Case coordination" means a mechanism for linking and | ||||||
| 20 | coordinating segments
of the service delivery system to ensure | ||||||
| 21 | the most comprehensive program
for meeting an individual | ||||||
| 22 | client's needs. It facilitates client movement
through an array | ||||||
| 23 | of services so that at any given time, services received are
| ||||||
| 24 | matched to the needs of the individual.
