AMENDMENT TO HOUSE BILL 4515
AMENDMENT NO. ______. Amend House Bill 4515, AS AMENDED,
with reference to page and line numbers of House Amendment No.
1, on page 18, line 21, after "waiver", by inserting the
following:
", including listing the specific disqualifying offenses for
which the waiver is being granted or denied. The Department
shall issue additional copies of this written notification upon
the applicant's, student's, or employee's request".
