S.F. 251 Section 1. DEPARTMENT OF PUBLIC HEALTH —— BREASTFEEDING 1 QUALITY IMPROVEMENT STRATEGIC PLAN. 2 1. The department of public health shall convene a group 3 of stakeholders from various sectors to develop an actionable 4 breastfeeding quality improvement strategic plan to determine 5 the framework necessary to maximize breastfeeding support 6 and improve the breastfeeding rate among all mothers in the 7 state. The strategic plan shall include goals and strategies 8 to promote breastfeeding at the state, community, facility, and 9 individual levels by identifying potential system interventions 10 and establishing a consistent approach relative to maternity 11 care and hospital practices; workplace, peer, mother and 12 family, and health care professional supports and education; 13 and media and social marketing messaging. The strategic plan 14 shall include steps to build collaborative partnerships and 15 local coalitions among health care providers and health care 16 facilities to follow updated evidence-based best practices, 17 policies, and procedures in supporting breastfeeding as the 18 preferred long-term infant feeding method. 19 2. In developing the strategic plan, the stakeholder 20 group shall review the state’s existing policies, practices, 21 and initiatives to maximize resources and to refocus and 22 coordinate efforts at the state, local, and organizational 23 levels. The stakeholder group shall analyze existing data 24 and make recommendations for strengthening the state’s data 25 collection and evaluation capabilities to support the strategic 26 plan, bridge gaps in disparities, build and sustain capacity, 27 promote professional and consumer education efforts, and 28 support continuous program improvement. The stakeholder group 29 shall also analyze other state and national laws, strategic 30 plans, initiatives, and programs to determine those which may 31 be replicated or modified to meet the goals and strategies 32 identified by the stakeholder group including but not limited 33 to all of the following: 34 a. S.F. 251 world health organization and the united nations children’s 1 fund to provide evidence-based maternity care that supports 2 optimal infant feeding outcomes and the mother-baby bond 3 utilizing the ten steps to successful breastfeeding to improve 4 breastfeeding awareness, education, and support within health 5 care settings. 6 b. The American academy of pediatrics guidelines which 7 promote exclusive breastfeeding for the first six months 8 followed by breastfeeding in combination with the introduction 9 of complementary foods for one year or longer as mutually 10 desired by the mother and infant. 11 c. The 2011 surgeon general’s call to action to support 12 breastfeeding specifying twenty action steps for mothers and 13 their families, communities, the health care sector, employers, 14 research and surveillance, and the public health infrastructure 15 to undertake in removing obstacles to and in supporting 16 breastfeeding. 17 d. The business case for breastfeeding initiative and 18 toolkit developed by the United States department of health 19 and human services maternal and child health bureau and the 20 national business group on health to implement policies that 21 promote breastfeeding. 22 e. The mothers’ milk bank of Iowa and other milk-sharing 23 platform initiatives. 24 f. The United States department of agriculture’s special 25 supplementary nutrition program for women, infants, and 26 children (WIC) program and the Medicaid program, to act as a 27 foundation in promoting breastfeeding awareness and to provide 28 skilled support not only for WIC and Medicaid participants 29 but for the general public to help women initiate and sustain 30 appropriate feeding practices, prevent difficulties, and 31 overcome barriers. 32 3. The department of public health shall submit the 33 strategic plan, which shall include goals, specific strategies, 34 action steps, policy changes, and key partnerships necessary 35 -2- LSB 1377XS (2) 89 pf/rh 2/ 3
S.F. 251 to execute the strategic plan, to the governor and the general 1 assembly by July 1, 2022. 2 EXPLANATION 3 The inclusion of this explanation does not constitute agreement with 4 the explanation’s substance by the members of the general assembly. 5 This bill requires the department of public health (DPH) 6 to convene stakeholders from various sectors to develop an 7 actionable breastfeeding quality improvement strategic plan to 8 determine the framework necessary to maximize breastfeeding 9 support and improve the breastfeeding rate among all mothers 10 in the state. The bill specifies that the strategic plan 11 shall include goals and strategies by identifying potential 12 system interventions and establishing a consistent approach 13 relative to maternity care and hospital practices; workplace, 14 peer, mother and family, and health care professional 15 supports and education; and media and social marketing 16 messaging. The strategic plan shall include steps to build 17 collaborative partnerships and local coalitions to follow 18 updated evidence-based best practices, policies, and procedures 19 in supporting breastfeeding as the preferred long-term infant 20 feeding method.
