Section
1.
INFANT
CERTIFICATE
OF
DEATH
FORM

INFORMATION

REGARDING
IMMUNIZATIONS.

1.
The
certificate
of
death
form
for
medical
examiner
3
cases
for
infants
zero
to
three
years
of
age
shall
include
4
a
request
for
information
regarding
the
date
and
type
of
the
5
decedent’s
last
immunization.
If
the
decedent
received
more
6
than
one
immunization
at
the
time
of
the
last
immunization,
the
7
information
provided
shall
include
all
types
of
immunizations
8
received.

2.
The
department
of
public
health
shall
adopt
rules
10
pursuant
to
chapter
17A
to
amend
the
certificate
of
death
form
11
for
medical
examiner
cases
for
infants
zero
to
three
years
of
12
age
to
comply
with
this
section.

EXPLANATION

The
inclusion
of
this
explanation
does
not
constitute
agreement
with
15
the
explanation’s
substance
by
the
members
of
the
general
assembly.

This
bill
requires
that
a
certificate
of
death
form
for
17
medical
examiner
cases
for
infants
zero
to
three
years
of
age
18
shall
include
a
request
for
information
regarding
the
date
and
19
type
of
the
decedent’s
last
immunization.
If
the
decedent
20
received
more
than
one
immunization
at
the
time
of
the
last
21
immunization,
the
information
provided
shall
include
all
types
22
of
immunizations
received.
The
bill
requires
the
department
23
of
public
health
to
adopt
administrative
rules
to
amend
the
24
certificate
of
death
form
for
medical
examiner
cases
for
25
infants
zero
to
three
years
of
age
to
comply
with
the
bill.
