1.
COLLECTION
OF
EMPLOYER
INFORMATION

IOWA

HEALTH
AND
WELLNESS
PLAN.
The
department
of
human
services
2
shall
require
an
applicant
for
or
recipient
of
Iowa
health
3
and
wellness
plan
coverage
to
provide
the
name
and
contact
4
information
of
the
applicant’s
or
recipient’s
employer
at
the
5
time
of
initial
application
and
at
any
time
the
recipient’s
6
employer
changes.
The
department
shall
compile
and
publish
on
7
the
department’s
internet
site,
on
a
quarterly
basis,
a
listing
8
of
those
employers
who
employ
Iowa
health
and
wellness
plan
9
recipients,
including
the
number
of
recipients
employed
by
each
10
employer.
For
the
purposes
of
the
compiling
and
publishing
11
of
information
by
the
department
of
human
services
under
this
12
section,
“employer”
means
a
person
who
employs
twenty
or
more
13
employees,
excluding
part-time
employees.

does

agreement

the
members
of
the
general
assembly.

This
bill
directs
the
department
of
human
services
(DHS)
18
to
require
an
applicant
for
or
recipient
of
Iowa
health
19
and
wellness
plan
coverage
to
provide
the
name
and
contact
20
information
of
the
applicant’s
or
recipient’s
employer
at
the
21
time
of
initial
application
and
at
any
time
the
recipient’s
22
employer
changes.
DHS
is
required
to
compile
and

the
department’s
