        Introduced  by  Sens.  SKOUFIS,  MAY,  RIVERA  -- read twice and ordered
          printed, and when printed to be committed to the Committee on Health

        AN ACT to amend the public health law, in relation to providing informa-
          tion on where a list of violations and other actions taken against the
          facility can be found

          The People of the State of New York, represented in Senate and  Assem-
        bly, do enact as follows:

     1    Section  1. Section 2803 of the public health law is amended by adding
     2  a new subdivision 13 to read as follows:
     3    13. The commissioner shall require each residential health care facil-
     4  ity to provide residents and their families with a separate document, as
     5  part of an intake application, in no less than twelve-point  font,  that
     6  includes  information  on  how  a  potential  resident  and their family
     7  members can look  up  complaints,  citations,  inspections,  enforcement
     8  actions,  and  penalties  taken  against  the facility including the web
     9  address for the New York state nursing home  profiles  website  that  is
    10  maintained  by the department and the nursing home compare website main-
    11  tained by the United States department of health and human services,  if
    12  applicable.
    13    §  2.  This  act  shall take effect on the sixtieth day after it shall
    14  have become a law.
