Bill Title: Amends the Adoption Act. Defines "adult grandchild" as the biological grandchild 21 years of age or over of a deceased adopted or surrendered person. Replaces references to the Adoption Advisory Council with references to the Adoption Registry-Confidential Intermediary Advisory Council. Makes changes in Sections concerning the Illinois Adoption Registry and Medical Information Exchange; the Adoption Registry-Confidential Intermediary Advisory Council; disclosure of identifying information; registry matches; the Illinois Adoption Registry Application; and forms. In the Section concerning confidential intermediaries, makes changes in subsections concerning: general purposes; eligibility of petitioner; the petition; confidential intermediary access to information; remedies in the event of a missing or lost original birth certificate; and duties of the confidential intermediary in conducting a search.

2 | AMENDMENT NO. ______. Amend House Bill 5949 on page 63, | ||||||
3 | line 15, by changing " or surviving spouse " to " surviving | ||||||
4 | spouse, or birth parent of an adult adopted person "; and
| ||||||
5 | on page 63, by inserting immediately below line 19 the | ||||||
6 | following:
| ||||||
7 | " (m) Beginning on January 1, 2015, any birth parent of an | ||||||
8 | adult adopted person named on the original birth certificate | ||||||
9 | may request a non-certified copy of the original birth | ||||||
10 | certificate reflecting the birth of the adult adopted person, | ||||||
11 | provided that: | ||||||
12 | (1) any non-certified copy of the original birth | ||||||
13 | certificate released under this subsection (m) shall not | ||||||
14 | reflect the State file number on the original birth | ||||||
15 | certificate; and | ||||||
16 | (2) if the Department of Public Health does not locate |

1 | the original birth certificate, it shall issue a | ||||||
2 | certification of no record found. "; and

3 | on page 67, by replacing lines 2 through 9 with the following:

4 | "fee shall be charged to any petitioner if the petitioner is an | ||||||
5 | adult adopted or
surrendered person and the sought-after | ||||||
6 | relative is a birth
parent who filed or who did not file a | ||||||
7 | Denial with the Registry
prior to January 1, 2011, or filed a | ||||||
8 | Birth Parent Preference
Form on which Option E was selected | ||||||
9 | after January 1, 2011 and
more than 5 years have transpired | ||||||
10 | since the birth parent filed
the Denial of Information Exchange | ||||||
11 | or Birth Parent Preference
Form on which Option E was | ||||||
12 | selected .".
