Amends the State Finance Act and Illinois Vehicle Code to provide for the issuance of Alzheimer's Awareness license plates. Creates the Alzheimer's Awareness Fund as a special fund in the State treasury. Provides that the applicant shall pay an additional fee of $25 for original issuance of the plates and at renewal. Provides that $15 of the additional fee at original issuance and $2 of the additional fee at renewal shall be deposited into the Secretary of State Special License Plate Fund, and that $10 of the additional fee at original issuance and $23 of the additional fee at renewal shall be deposited into the Alzheimer's Awareness Fund. Provides that moneys in the Alzheimer's Awareness Fund shall be paid as grants to the Alzheimer's Disease and Related Disorders Association, Greater Illinois Chapter, for Alzheimer's care, support, education, and awareness programs.

| 4 | | Section 5. The State Finance Act is amended by adding |
| 5 | | Section 5.826 as follows:

| 11 | | Sec. 3-699. Alzheimer's Awareness license plates. |
| 12 | | (a) The Secretary, upon receipt of an application made in |
| 13 | | the form prescribed by the Secretary, may issue special |
| 14 | | registration plates designated as Alzheimer's Awareness |
| 15 | | license plates. The special plates issued under this Section |
| 16 | | shall be affixed only to passenger vehicles of the first |
| 17 | | division and motor vehicles of the second division weighing not |
| 18 | | more than 8,000 pounds. Plates issued under this Section shall |
| 19 | | expire according to the multi-year procedure established by |
| 20 | | Section 3-414.1 of this Code. |
| 21 | | (b) The design and color of the plates is wholly within the |

| 1 | | discretion of the Secretary. The Secretary may allow the plates |
| 2 | | to be issued as vanity plates or personalized under Section |
| 3 | | 3-405.1 of this Code. The Secretary shall prescribe stickers or |
| 4 | | decals as provided under Section 3-412 of this Code. |
| 5 | | (c) An applicant for the special plate shall be charged a |
| 6 | | $25 fee for original issuance in addition to the appropriate |
| 7 | | registration fee. Of this fee, $10 shall be deposited into the |
| 8 | | Alzheimer's Awareness Fund and $15 shall be deposited into the |
| 9 | | Secretary of State Special License Plate Fund, to be used by |
| 10 | | the Secretary to help defray administrative processing costs. |
| 11 | | For each registration renewal period, a $25 fee, in |
| 12 | | addition to the appropriate registration fee, shall be charged. |
| 13 | | Of this fee, $23 shall be deposited into the Alzheimer's |
| 14 | | Awareness Fund and $2 shall be deposited into the Secretary of |
| 15 | | State Special License Plate Fund. |
| 16 | | (d) The Alzheimer's Awareness Fund is created as a special |
| 17 | | fund in the State treasury. All money in the Alzheimer's |
| 18 | | Awareness Fund shall be paid, subject to appropriation by the |
| 19 | | General Assembly and distribution by the Secretary, as grants |
| 20 | | to the Alzheimer's Disease and Related Disorders Association, |
| 21 | | Greater Illinois Chapter, for Alzheimer's care, support, |
| 22 | | education, and awareness programs.
