H.F. 2032 Section 1. NEW SECTION . 135.163A Public health advancement 1 commission. 2 1. a. A strong and effective public health system is 3 essential to meet the obligations of public health agencies to 4 protect and improve the population’s health and to respond to 5 crises and ongoing challenges. 6 b. A comprehensive health policy is necessary to align 7 governmental public health agency structure, function, and 8 administration, health-sector investment, and private sector 9 involvement to work more effectively as part of a broader 10 public health system. 11 c. The public health infrastructure should act as the 12 clear organizational hub for public health activities to 13 provide better linkages to other governmental agencies 14 with health-related responsibilities and to strategies that 15 strengthen the capacities of public health agencies to perform 16 core functions. 17 2. The department shall establish a public health 18 advancement commission to develop and maintain an effective 19 and efficient public health system based on the core functions 20 of public health and the provision of essential services 21 to all Iowans by monitoring and addressing public health 22 infrastructure, priorities, capacity, and financing. The 23 department shall involve the wide range of professionals, 24 volunteers, agencies, and organizations engaged in 25 health-related responsibilities and public health activities 26 in the work of the commission. 27 3. The purposes of the public health advancement commission 28 shall include all of the following: 29 a. To provide leadership and serve as a resource to foster 30 and maintain an effective and efficient public health system. 31 b. To convene stakeholders and streamline efforts to 32 encourage collaboration and synergy between community partners 33 in reducing fragmentation and supporting state-level and 34 community-level integration to most effectively and efficiently 35 -1- LSB 5215YH (2) 89 pf/rh 1/ 5
H.F. 2032 address social determinants of health and to meet population 1 health goals. 2 c. To provide a venue for educating and informing 3 stakeholders and interested parties about the role of public 4 health. 5 4. The duties of the commission shall include all of the 6 following: 7 a. Assessing the current public health infrastructure to 8 determine if and how the core functions of public health and 9 essential public health services are being carried out and 10 provided in the state and to determine the current inventory, 11 baseline, and mapping of public health services and how they 12 are being funded to determine gaps and unmet needs. 13 b. Assessing existing policies, systems, and factors 14 relating to the core functions of public health and the 15 delivery of essential public health services to determine 16 opportunities for improvement. 17 c. Reviewing and guiding strategic state-level planning 18 and policies related to public health infrastructure, roles 19 and responsibilities, and system assessment and performance to 20 align the public health system on the state and local levels to 21 ensure the delivery of essential public health services in each 22 area of the state. Such alignment shall focus on improving 23 the administration of programs and policies to reflect and 24 satisfactorily address the specific needs of local communities 25 and vulnerable populations and to eliminate health disparities; 26 promoting primary prevention and early intervention to improve 27 the overall health status of Iowans; and addressing the myriad 28 of current and future threats to public health by establishing 29 clear lines of responsibility, coordination, accountability, 30 and communication. 31 d. Developing an inventory of public and private resources 32 that support public health, and through coordination, 33 collaborative planning, and leveraging of public health and 34 private assets, link and strengthen both governmental and 35 -2- LSB 5215YH (2) 89 pf/rh 2/ 5
H.F. 2032 nongovernmental roles that comprise the public health system at 1 the state and local levels. The commission shall identify and 2 explore other public and private funding sources that support 3 the delivery of public health services and shall review and 4 implement a process to braid public health resources together 5 to effectively reach the highest-risk populations, provide 6 for more efficient program administration, and ensure that 7 essential public health services are delivered to all Iowans 8 throughout the state. 9 e. Implementing a local public health infrastructure that 10 utilizes the strengths of the existing network of public 11 health organizations and community partners to streamline 12 administration, increase local community capacity, and ensure a 13 more coordinated system for delivery of essential public health 14 services through multisector community health coalitions. The 15 community health coalitions shall engage representatives that 16 reflect the local community including those who represent 17 vulnerable populations, those most at risk, and those with the 18 necessary expertise to assess health and quality of life needs 19 and assets, create and coordinate plans to address identified 20 needs, and mobilize resources to implement those plans. 21 The commission shall develop core competencies, functions, 22 indicators, and performance standards for the community health 23 coalitions. 24 f. Reviewing the public health systems in other states 25 and countries to inform changes in the public health 26 infrastructure; determine best practice models and options for 27 a comprehensive statewide public health framework, including 28 a communications infrastructure; and compare how comparable 29 public health functions are structured statewide and at the 30 substate levels. 31 g. Creating an interdepartmental subcommittee to inventory 32 resources and develop a plan to integrate responsibilities 33 and funding sources to support public health priorities and 34 functions in the state. Each member agency of the subcommittee 35 -3- LSB 5215YH (2) 89 pf/rh 3/ 5
H.F. 2032 shall be tasked with determining how to integrate the core 1 competencies, functions, and performance standards of the 2 agency into the work and funding of the public health system. 3 h. Developing a communications strategy to utilize in 4 communicating with local public health agencies and community 5 partners, as well as the general public. 6 i. Developing and implementing data reporting tools and data 7 analysis to provide a basis for policy development, performance 8 measures, and accountability. 9 j. Providing ongoing technical assistance to local public 10 health entities and community health coalitions. 11 k. Addressing the need for an adequately sized and 12 appropriately trained workforce to respond to the array 13 of public health needs, including by identifying public 14 health workforce needs and addressing workforce training and 15 competency. 16 5. a. The membership of the commission shall be selected by 17 the director and shall include representatives of all of the 18 following: 19 (1) Local public health agencies. 20 (2) State agencies involved in protecting and improving 21 population health. 22 (3) The Iowa public health association. 23 (4) Emergency management agencies. 24 (5) Emergency medical services. 25 (6) School districts. 26 (7) Institutions of higher education, including but not 27 limited to the university of Iowa college of public health. 28 (8) Health care providers, health clinics, and community 29 health centers. 30 (9) Area agencies on aging and senior services providers. 31 (10) Mental health and disability services regions. 32 (11) Other community-based organizations, issue specific 33 coalitions, and civic groups. 34 (12) Environmental advocacy. 35 -4- LSB 5215YH (2) 89 pf/rh 4/ 5
H.F. 2032 (13) Planning, zoning, and transportation agencies. 1 (14) Organizations, programs, and advisory groups involved 2 in the healthy Iowans planning and implementation efforts. 3 b. The commission may utilize the assistance of others with 4 relevant public health-related expertise when necessary to 5 carry out the commission’s responsibilities. 6 6. The commission shall submit a report to the governor 7 and the general assembly annually by January 15, including 8 recommendations for improvement in the public health system. 9 EXPLANATION 10 The inclusion of this explanation does not constitute agreement with 11 the explanation’s substance by the members of the general assembly. 12 This bill creates the public health advancement commission. 13 The bill directs the department of public health to 14 establish the commission to develop and maintain an effective 15 and efficient public health system based on the core functions 16 of public health and the provision of essential services 17 to all Iowans by monitoring and addressing public health 18 infrastructure, priorities, capacity, and financing. The 19 department shall involve the wide range of professionals, 20 volunteers, agencies, and organizations engaged in public 21 health activities in the work of the commission. 22 The bill specifies the purposes, duties, and membership of 23 the commission, and requires the commission to submit a report 24 to the governor and the general assembly annually by January 25 15, including recommendations for improvement in the public 26 health system. 27 -5- LSB 5215YH (2) 89 pf/rh 5/ 5
