Requires the application form for motor vehicle registration or renewal of registration to include language permitting the applicant to make a voluntary contribution to Prevent Child Sexual Abuse. Requires the application form for a driver's license or duplicate thereof to include language permitting the applicant to make a voluntary contribution to Prevent Child Sexual Abuse or to Prevent Blindness Florida, etc.
